Helping People Stay Healthy and Housed with Whole-Person Care

The San Francisco Department of Public Health improved care coordination to provide a stronger safety net for people experiencing homelessness.

San Francisco Department of Public Health’s Whole Person Care program helps clinicians refer patients to housing resources by integrating shelter and other census data directly into Epic. Using Healthy Planet Link, Epic’s web-based portal for community-based organizations, and Compass Rose, Epic’s application for comprehensive care coordination, these organizations can access medical information about unhoused patients when and where they need it. The program, which started as a small pilot, has now become an established, ongoing project to increase care coordination across San Francisco’s care providers.

How They Did It:

  • Added shelter census information to Epic
  • Improved documentation accessibility for community-based organizations using Healthy Planet Link
  • Tracked program participants with Epic’s application for comprehensive care coordination, Compass Rose, to follow up on care and services
  • Used grants to fund holistic treatment for at-risk populations
