ACT Health added a link in MyChart to the national Australian Organ Donor Register to help people document their donation wishes. Nearly 12,300 people clicked the link in 2024, making MyChart among the most-used methods to bring people to the website across the country. The conversion rate—the percentage of people who then signed up to become a donor—was 30%, compared to 8% for other recruitment methods.

Malnutrition leads to higher mortality and readmission rates, and while up to 50% of hospitalized adults in the U.S. are malnourished, only 5-8% are diagnosed. UI Health Care, UW Health, and MaineHealth implemented documentation improvements and fostered cross-team collaboration to improve malnutrition diagnosis and treatment in their hospitals. Through more accurate identification of malnutrition in admitted patients, two organizations saw an 8-12% increase in diagnosis rates and one reported $650,000 in additional annual reimbursement to cover the increased cost of care for malnourished patients. Their efforts serve as a model for reporting to CMS on the Global Malnutrition Composite Score.

Predictive Models in Practice: Case Studies on the Deterioration Index

Hospitals using a predictive model that identifies patients at risk of deterioration to prompt interventions reported decreasing mortality by up to 17%, with RWJBarnabas Health estimating it contributed to more than 500 lives saved since early 2023 and North Memorial Health estimating 126 lives saved annually since full health system adoption. In combination with updated processes, risk scores led to secondary benefits like decreasing disruptive overnight vital checks at Lehigh Valley Health Network and improving end-of-life care at RWJBarnabas Health.

The program supports primary care clinicians in providing the parents of children ages 0-5 with guidance about the importance of building healthy relationships and encouraging parents to make reading with their child part of their daily routine. Healthcare organizations that participate in Reach Out and Read can use Epic to support conversations about shared reading.
